DATA PRIVACY CONSENT

I acknowledge that I have read, understood and hereby agree to the Data Privacy Statement of Philippine Life Financial Assurance Corporation ("PhilLife"), a copy of which has been provided to me, pursuant to the Data Privacy Act of 2012, its implementing rules, regulations, circulars and issuances by the National Privacy Commission ("Privacy Laws").

I acknowledge and consent to the collection, use, disclosure, transfer, retention, and disposal (collectively "Processing") by PhilLife of my Personal Information and Sensitive Personal Information (collectively "Personal Data") which are provided in this form strictly for purposes relevant to the employer-employee relationship between me and PhilLife, particularly for the processing of my employee benefits and transactions with relevant government agencies. I have been duly advised that my Personal Data shall not be processed for any other reason and be kept secure in the strictest confidence.

I represent and warrant that consents required under the Privacy Law were obtained from other individuals whose Personal Data I have shared to PhilLife whenever necessary in the course of my employment.
